The vision of 2040 Partners for Health is a world of healthy communities that elevate and maximize human potential and well-being. 2040 was initially launched as a project in 2006 when local long-time health advocates, educators and public service professionals envisioned the need to foster communities’ resources, desires and commitments as mutual partners in the seven neighborhoods of Northwest Aurora, East Montclair, Greater Park Hill, Northeast Park Hill, Stapleton, Montbello and Commerce City. These communities are home to resident populations with a long history of marginalization and significant health and economic disparities with a broad array of population characteristics, diversity and culture.

Using community-based, participatory approaches 2040 implements research and programs in collaboration with community residents, non-profit organizations, healthcare service providers as well as the faculty researchers and students at the University of Colorado. 2040 Cultivates partnerships and advances community solutions to tackle critical health needs at the neighborhood level and facilitate progressive action. Over the last 12 years, 2040 Partners for Health has distinguished itself through its connectivity to the community and building a strong foundation of working together to improve health equity for the long term.
