Our Communities

2040 Partners for Health serves the neighborhoods of East Montclair, Greater Park Hill, Northeast Park Hill, Northwest Aurora, Montbello, Commerce City and Stapleton. These communities are home to resident populations with a long history of marginalization and health disparities, but also filled with valuable assets, a wealth of diversity, strengths and committed community members.
